BACKGROUND: Oligo-astheno-teratozoospermia (OAT) syndrome is one of the causes of male infertility, characterized by a decreased number of sperms (oligospermia), poor sperm motility (asthenospermia), and abnormal sperm shape (teratospermia). In necrospermia, spermatozoa in the ejaculated semen are dead, and in hypospermia, semen volume is less than 2 mL. The present case may be unique because of the association of all these features together in a single patient. METHOD: This idiopathic case was treated, from November 2016 to April 2017 in the National Homeopathy Research Institute in Mental Health, with the homeopathic medicine Natrium muriaticum. The Modified Naranjo Criteria for Homeopathy (MONARCH) score was used to assess the likelihood of causal attribution of the clinical outcome to the homeopathic medicine. RESULT: By the third month of starting homeopathic treatment, there was a marked improvement in sperm count from 14.4 to 77.4 million, semen volume from 0.8 to 2.2 mL, and normal forms from 14 to 75%. His wife became pregnant by the 5th month and delivered a healthy baby girl at full term. A MONARCH score of +9/13 indicates a positive causal relationship between the medicine prescribed and the treatment outcome. CONCLUSION: The present case report suggests a positive effect of individualized homeopathic medicine in treating a rare presentation of male infertility due to OAT syndrome.

BACKGROUND: Ovarian masses may be either physiological or pathological. Physiological ovarian masses are non-neoplastic. Pathological masses grow more than 7 cm and persist beyond 3 months; they do not resolve spontaneously, and invasive surgery with removal of reproductive organs is the conventional treatment. It usually results in further co-morbidities and hampers the quality of life of the patient. OBJECTIVES: This case series of pathological left ovarian masses demonstrates the role of homeopathic treatment in each of two multiparous women where the right ovary was surgically removed previously for similar pathology and in an adolescent girl with increased tumor marker CA-125. METHODS: Three cases of women with pathological ovarian masses in the left ovary, and advised invasive surgery by attending gynecologists, were treated with homeopathic medicines at the outpatient department of the National Homeopathy Research Institute in Mental Health, Kottayam, Kerala, from 2017 to 2020. Each case was followed up with clinical and ultrasonographic evidence and reported according to the criteria set out in the HOM-CASE guidelines. RESULT: Complete resolution of the ovarian masses was observed in all three cases, evidenced by ultrasonography, with normalizing CA-125 values also observed in the case of endometrial cyst. The patients improved within 4 to 15 months of treatment using Thuja and other individualized homeopathic medicines. The MONARCH score was calculated as +7/13, +9/13, and +9/13 for the three cases, respectively, indicating a positive causal attribution of homeopathy in the resolution of these pathological ovarian masses. CONCLUSION: This case series suggests that individualized homeopathy, and notably Thuja, can be useful in the treatment of pathological ovarian masses.

BACKGROUND: Polar symptoms (PS)-symptoms with opposite values-are frequently used in homeopathy, but have many misleading entries in the repertory. This is caused by using absolute occurrence of symptoms, causing the same medicine to appear in both (opposite) symptom rubrics, and by lack of comparison with other medicines. Some PS, like 'aversion/desire for sweets' have a frequency distribution that is not evenly distributed around the neutral value: a desire for sweets is much more common than aversion. A desire for sweets is an indication for a specific medicine only if this desire occurs more frequently in this specific medicine population than in the remainder of the population. We need to find the best way to represent this difference. METHODS: A multi-centre, explorative, prospective, observational study was conducted by nine centres of the Central Council for Research in Homoeopathy. Two-hundred and sixteen patients were enrolled with chronic cough lasting more than 8 weeks, and received usual homeopathic care. During intake, 30 general PS, 27 polar cough symptoms and 3 non-polar cough symptoms were checked. Different ways of representing results were explored, including two quantities borrowed from mechanics: Centre of Mass (CoM) and Leverage. RESULTS: At the fourth follow-up, three medicines with more than 10 cases with good results were identified: 20 Phosphorus, 19 Pulsatilla and 13 Sulphur. The mean value of the frequency distribution of some symptoms in the whole sample was considerably different from the neutral value. Comparing a medicine population with the remainder of the respective population can give results that differ from polarity analysis. For some symptoms, the 'distance' (Leverage) between the CoMs of the medicine population and the remainder of the population was clearer than the likelihood ratio (LR). CONCLUSION: If the LR value is not clear about the prognostic value in PS, notions from mechanics such as CoM and Leverage can clarify how to interpret a polar symptom.
